Prepare For An Appointment
Prior to your appointment you will receive information by mail which includes a patient questionnaire. Patients are requested to complete and return the questionnaire prior to their scheduled appointment. The completed questionnaire should be returned to Vendetta Matthews at the Johns Hopkins Hospital, Pathology Building, Room 2-210, 600 N. Wolfe Street, Baltimore, MD 21287. You should also bring a copy of the completed questionnaire with you on the day of your appointment.

On the day of your appointment, please report to the Johns Hopkins Outpatient Center, 601 N. Caroline Street. You will register for your appointment through the Neurology Clinic on the 5th floor. Please arrive for registration at least 30 minutes before your appointment. Although you will register on the 5th floor, you will be seen in clinic on the 6th floor. Please plan to spend up to 4 (four) hours with us for your evaluation.
Please keep in mind the following:
It is likely that you will be tested by recording the movements of yours eyes. Do not wear any makeup on the skin around the eyes.
If you wear glasses, bring them with you.
Bring (or have sent) ALL prior medical records, the actual X-rays and MRI scans, and the test results related to the problem for which you are being seen.
Do not eat a heavy meal prior to your appointment.
It is necessary that we have a letter of referral from your doctor.
Insurance Verification and Necessary Referrals
If you do not have health insurance with
Blue Cross/Blue Shield, Medicare, or have an HMO approval, we must ask you to pay your entire bill at the time of your visit (we accept credit cards). Please
contact your insurance company or our financial counselor (410.955.6486) to
find out if you are covered for your visit.
If your insurance company requires you to obtain a referral, please fax it to Vendetta Matthews prior to your appointment at 410.614.1746.
