Prepare For An Appointment

Patients who have been contacted for an appointment within our Center will receive a new patient package in the mail. We ask that you complete the new patient questionnaire in that packet prior to coming to your scheduled visit and bring it with you to your appointment. You should also bring any documents, films or CDs, or any other information not included in your medical record.
