Prepare For An Appointment

Prior to your visit, please make sure that all medical records, test results, etc. be sent/faxed to us. If you plan to fax these documents to the office, please fax to 410.614.2297:

Any films available (MRI, Cat Scans, etc) should brought with the patient to the visit. If a referral is needed, please have your Primary Care Provider’s office fax it to us at 410.614.2297 at least seven days prior to the appointment.

We also recommend that you call your doctor’s office one week prior to the visit to confirm the patient is coming and that we have everything we need -- especially the referral and/or insurance authorization.
