Prepare For An Appointment

Prior to your scheduled appointment, you will receive a letter confirming your appointment and a new patient questionnaire in the mail. Please fill out the questionnaire and bring it with you to your appointment. You may also download the questionnaire by clicking the link on the right hand side of this page. We also ask that you bring:

• A list of your current medications, including dosage and reason for taking each
• Any additional medical records you have not already sent to us, relevant to your neurological diagnosis or symptoms
• If you have radiology films, please bring them with you to the appointment. You do not need to send these to us beforehand.

Insurance Verification and Referrals

We recommend that you contact your insurance company prior to your appointment to verify benefits for your office visit, lab work, radiology, and other potential services.

If your insurance company requires a referral, it should be faxed to your physician’s office prior to the appointment.
