Prepare For An Appointment

Prior to your scheduled appointment, you will receive the following information in the mail:

• A letter confirming your appointment
• A new patient questionnaire
• Other helpful information, such as maps and directions

Please fill out the questionnaire and bring it with you to your appointment. We also request that you bring:

• Your insurance card
• Referral or authorization number, if necessary
